Beebe Healthcare is relocating its Coordinated Care Center from Lewes to Long Neck, reflecting the need for expanded services on the Route 24 corridor.
The move is being made possible by a $1.8 million federal grant. Delaware's congressional delegation of Sen. Chris Coons, Sen. Lisa Blunt Rochester and Rep. Sarah McBride were on hand to celebrate the announcement Sept. 8.
The center will be located in a repurposed Beebe medical building at 32060 Long Neck Road. A map of the floor plan shows exam rooms, a lab and other facilities.
The Coordinated Care Center is a place for patients who do not have a primary care or specialty doctor to go after an emergency room visit or hospital stay, according to Dr. David Tam, Beebe president and CEO.
"The Coordinated Care Center is designed specifically to help people who are at their most vulnerable," Tam said. "They go to the ER, and what happens after that? How do we make sure they don't fall through the cracks and end up back in the hospital or ER?"
Beebe opened the Coordinated Care Center at 1305 Savannah Road in Lewes in September 2024.
As an example, Dr. Kristie Zangari, medical director of Beebe's hospitality service and chair of the Department of Medicine, said a diabetic patient was able to get an appointment there 48 hours after discharge from the hospital. She had her blood sugar and insulin reviewed, received affordable supplies and met with a diabetic nurse educator.
"By expanding the center to Long Neck, we can reach more patients, reduce barriers to care and create better health outcomes for the community," Zangari said.
Coons pointed out that the delegation secured $1.5 million for the Beebe family practice building that opened across the parking lot in 2025.
He said the new center will help ease what he called the rural healthcare crisis.
"Attracting, training and retaining the workforce that we need is one of the top things I hear from central and western Sussex," Coons said. "This federal investment is the next stage of delivering healthcare that is sustainable, cost-effective, humane and effective."
The new center in Long Neck is expected to open next summer.
