La Red Health Center recognized as a Patient Centered Medical Home

March 18, 2013

La Red Health Center has been recognized by the National Committee for Quality Assurance as a Patient Centered Medical Home.

A PCMH is a model of care where patients have a direct relationship with a provider who coordinates a cooperative team of healthcare professionals, takes collective responsibility for the care provided to the patient and arranges for appropriate care with other qualified providers as needed. Designed to improve primary care through a set of standards that describe clear and specific criteria, a PCMH organizes care around patients and tracks an individual’s care over time.

The standards include access and communication, patient tracking and registry functions, care management,  self-management support, electronic prescribing, test tracking, referral tracking, performance reporting and improvement, and advanced electronic communications.

The delivery of quality healthcare is further enhanced by registries, information technology, health information exchanges and other means to ensure that patients get the indicated care when and where they need it in a culturally and linguistically appropriate manner.

Started by a network of physicians in 2001 to meet the needs of a growing number of locally uninsured individuals, LRHC is now a Federally Qualified Health Center whose mission is to be a center of excellence which provides quality, patient-centered care to the diverse members of the community.

La Red Health Center has two convenient locations: Family Practice/Prenatal/Mental Health/Oral Health, 21444 Carmean Way, Georgetown, 302-855-1233; and Family Practice, 1340 Middleford Road, Suite 401, Seaford, 302-628-7752.
