No Beneficiary on IRA at Death, Now What??

You have an IRA or a retirement plan. You worked hard for that money. It should go to the beneficiary you select. Did you select one? Does the company still have the form?

IRAs pass by a beneficiary form, not by a will. When there is no beneficiary form, or if the named beneficiaries have predeceased the account owner, or if the company misplaces your form (which happened to me believe it or not) then who does the IRA go to?
