Bayhealth Launches New Congestive Heart Failure Clinic to Reduce Readmissions and Improve Patient Outcomes

Bayhealth Heart & Vascular Institute has launched a new Congestive Heart Failure (CHF) Clinic designed to provide timely follow-up and specialized care for patients recovering from a heart failure hospitalization. The clinic is the first and only dedicated heart failure clinic in Kent and Sussex counties. Operating out of both Bayhealth Cardiology, Dover, Governors Avenue and Bayhealth Cardiology, Milford, Sussex Avenue, it aims to improve patient outcomes during the vulnerable post-discharge period and reduce preventable emergency department visits and hospital readmissions.

Patients with a heart failure diagnosis will be scheduled for an expedited visit at the CHF Clinic after discharge. Each appointment includes evaluation by a cardiology specialist and a pharmacist, who collaborate to optimize medications, identify early signs of worsening symptoms and provide education to help patients safely manage their condition at home.

The clinic also offers IV diuretic therapy, an outpatient treatment that helps remove excess fluid and reduce strain on the heart. This is an important option for patients who might otherwise require hospitalization for symptom relief.
